First visit only. Three pages: contact information and ID, health history, and consent with photo release. Driver's license numbers and other sensitive ID details are not collected on this website — preview and download only. It is the client's responsibility to inform the artist of any medical or health/safety related changes that may arise after the initial packet is completed.

Filled at each booking with that session's reserved hours, the 40% deposit amount, and the applicable last-minute / no-show dollar total. A blank copy is available here for reference.
